Implementing value-based healthcare using a digital health exchange platform to improve pregnancy and childbirth outcomes in urban and rural Kenya

A case study on the development and implementation of a VBHC digital care bundle, called MomCare, in Kenya.

Created by the international NGO PharmAccess Foundation (PAF), the care bundle aims to improve maternal, neonatal and child health (MNCH) outcomes in low- and middle-income countries (LMICs) by addressing the financial and geographical barriers to access skilled delivery and incentivising adherence to antenatal guidelines.

The framework is based on VBHC; aiming to support development of comprehensive outcome measurements and lead to reimbursement systems that incentivise providers to maximise value. The implementation of MomCare is ongoing and the evaluation of health outcomes is ongoing, but initial results show improved adherence to evidence-based care pathways at a predictable cost per enrolled person. The incremental, cohort-based approach used in the implementation of MomCare has enabled iterative learning processes, which can support the restructuring of health systems in low resource settings from an output-driven model to a value-based financing-driven model.
